Important Immunization Information for Grade 9 & 10 parents

This school year, we will be running our usual immunization clinics for our Grade 9’s at the school. We will also be including the Grade 10’s who missed their immunizations last year.

 

There will be two clinics this year:

1.       the first on December 8th (Wednesday)

2.       the second on March 1st (Tuesday) *Tentative*

 

Consent packages will be sent home this week, November 16 - 19. Due to time constraints this fall term, there will be no follow up phone calls if consents are not returned. Students will only be immunized with parental consent.

 

If your child does not receive a consent form, it is because:

·         they are already complete for their age OR

·         they are already in progress getting vaccines elsewhere

 

If they had vaccines elsewhere, please kindly attach immunization histories. These include vaccines done at:

·         doctors office

·         pharmacy

·         travel clinic

·         different province/ country 

·         different health authority (i.e. Fraser Health)


Please read the pink form and the consent form very carefully. I will be picking up consent forms on Wednesday, November 24th.

 

If you are consenting to vaccines, please ensure you have ticked the box for the vaccines you want given, signed, dated the form and indicated that you are the parent or guardian.

 

Covid 19 vaccine, will also be offered to any students who turn 12 by the end of this year, but again, only with parental consent.

 

On the day of the clinic, December 8th, please ensure that your child wears a short sleeved shirt and has breakfast/ snack on the day of the clinic.
