Medical Action Plan forms (students with a Life-Threatening Medical alert)
If your child has a Life-Threatening Medical Alert, please fill out the form(s) below so that we know what to do in an emergency! Please ask your child's medical practitioner to sign it, and bring it to the office along with any medication to keep in the office.

Medication at school
Even if the condition is not life-threatening, we can keep medication at the office. Please fill out this form to tell us when and how we should give this medication to your student. Please ask your child's medical practitioner to sign it, and bring it to the office, along with the medication.
