Medical Alerts & Allergies

If your child has a life-threatening medical alert, please report the necessary information to the office immediately by updating the verification form that will be in the package. 

You will also be required to complete the permission form provided by the office with medications or EpiPens. Please be sure to check expiry dates before sending them to school.

David Livingstone Elementary School is allergy-aware! Please DO NOT send food containing nuts or peanut products to school with your child. We have some students with severe allergies to these particular foods. We cannot guarantee a nut-free environment. If your child does have an allergy, please help them to ensure their own safety by not sharing food with others at school.
