As we are moving towards a greener practice, the VSB has digitized several consent forms.  These forms include: 

• VSB Internal Use Media Consent
• External Use Media Consent
• CASL - Canadian Anti-Spam Legislation
• Adobe CC Consent 
• Student AUP - Acceptable Use Policy
• Neighbourhood Walk 

How to access forms: 

1. Go to https://myforms.vsb.bc.ca/
2. How to log in:
   • Enter student PEN (Nine-digit number assigned to you when you entered our school system) and birthdate. OR 
   • Student VSB email (7-digit-student-#@learn.vsb.bc.ca) and password. 
3. Accept the consent for each of the forms. 
   • Consent is given via a check box "I Agree". 
   • Not accepting or completing implies decline.
4. Once consent is given, a REVOKE button is exposed if you no longer want to give consent.  
   • If you do REVOKE, please email oppenheimer@vsb.bc.ca
