This form is to be completed for any student expecting to return to school LATER THAN noon on Friday September 8, 2023 but prior to Friday, September 29, 2023. Please note: If the school cannot verify your child’s attendance at school by noon on Friday September 8, 2023, your child’s space will be given to another student on the school’s waitlist.

Students are given out forms in homeroom on Wednesday September 6th. Please make sure that these forms are filled out, signed and returned to the trays in front of the main office as soon as possible.

The forms are:

1) Emergency Medical Information Sheet

2) PW Informed Consent for Local Community Activities

3) PW Data Collection Form

4) Emergency Reunification Form

Online forms: Please visit http://myforms.vsb.bc.ca and fill out the other forms online. 

1) VSB Media Consent

2) External Media Consent Form

3) Grade 4-12 AUP Form (Acceptable Use of Technology Policy) - very important, read through this carefully with your child.

4) Adobe CC Consent Form

5) CASL Consent Form (Communicating 

When logging into http://myforms.vsb.bc.ca please make sure you are entering your child's PEN and not their student number. Alternatively, you can login by clicking on "VSB staff and student sign in here" and entering your child's VSB email address and password.
