Please ensure the following forms are completed for your student. 

1. https://myforms.vsb.bc.ca/    You will require your student's Personal Education Number (PEN) and your student's birthdate to log in.  The PEN can be found in MyEd or on a past report card. 
2. Student Verification forms:    Student Verification Form can be found on the MyEd BC Family Portal.  Please follow the instructions below to locate the form. 
   1. Go to the MyEd Website(https://www.myeducation.gov.bc.ca/aspen/logon.do)
   2. Type in your student’s student number for the login.
   3. Your password is the one that you and your student have already been using.  
   4. You will find your child’s student verification form under “Published Reports” on the right-hand side.
